Referral data recorded on database and file allocated to a project lead.

Home visit arranged to assess suitability.

Communication with professionals and parents/carers re support service considered to be most suitable, ie. club or befriender.

Invite to club and arrange transport if necessary. Or allocate a suitable befriender and arrange match.

Targets are set for the young person to work towards during their time at club or with their befriender.

Regular work with the young people either at clb or during befriending matches with regular reviews.

Young person leaves the project having achieved their goals.
